Pulmonology​

Pulmonology or pneumology is a medical specialty that deals with diseases involving the respiratory tract. It is also known as respirology, respiratory medicine, or chest medicine in some countries and areas. Pulmonology is considered a branch of internal medicine, and is related to intensive care medicine. Pulmonology often involves managing patients who need life support and mechanical ventilation.

Across the world, there is always confusion as to which is better - heat or ice application.

  • Iron Isomaltoside (Monoferric / Ferric Derisomaltose): Intravenous iron replacement therapy for iron deficiency anemia (IDA) in adults who are intolerant or unresponsive to oral iron therapy. It consists of iron and isomaltoside (a carbohydrate) forming a controlled-release matrix structure. Available as 100 mg/mL elemental iron in 1 mL, 5 mL, and 10 mL vials.

    • Mechanism: Iron is vital for formation of new red blood cells and hemoglobin, enabling oxygen transport capability.
    • Indications: Treatment of iron deficiency, iron deficiency anemia, anemia due to chronic kidney disease (CKD).
    • Administration: Must be administered under trained healthcare professional supervision only. Dosing calculated using Ganzoni formula (based on patient weight, iron stores, target/actual hemoglobin) or simplified tables (1,000-2,000 mg doses).
      • Bolus injection: Up to 500 mg at 250 mg/minute rate, maximum once weekly
      • Infusion: If cumulative dose exceeds 20 mg iron/kg body weight, split into two administrations at least one week apart. Single doses above 1,500 mg not recommended.
    • Common Side Effects: Vomiting, nausea, dark colored stool, constipation, diarrhea
    • Contraindications/Precautions:
      • Not recommended during pregnancy (positive evidence of fetal risk from animal studies)
      • Safe during breastfeeding (does not pass into breast milk in clinically significant amounts)
      • Unsafe with alcohol consumption
      • Use with caution in liver disease (dose adjustment may be needed)
      • Kidney disease: Limited data, consult doctor
    • Drug Interactions: Compatible with vitamin C, vitamin D, multivitamin, folic acid, paracetamol. Should NOT be taken with zinc (alters zinc absorption).
    • Monitoring: Continue until hemoglobin levels normalize. May cause weight gain.
    • Cost: Drug acquisition cost per course of therapy is 450to450 to 900 (U.S. pricing).
