Airplane Ear (Barotrauma) & Eustachian Tube Pressure
Airplane ear (ear barotrauma) is the painful pressure buildup that happens when the middle ear can't equalize fast enough with the rapid cabin pressure changes during descent.
Why It Happens
- The Eustachian tube connects the back of the throat to the middle ear and has to open to equalize pressure as the cabin pressurizes/depressurizes.
- If the nasal passages or tube are already swollen (a cold, sinus infection, allergies, or a Deviated Nasal Septum), the tube can't open easily, trapping pressure and fluid in the middle ear.
- Repeated or severe barotrauma can inflame the eardrum further, and combined with an active sinus infection, can escalate into a middle ear infection (otitis media).
- Airplane cabin air is also extremely dry, which dries out protective nasal/throat moisture and makes it easier for viruses to take hold — a common reason people fall sick a few days after a long flight (typical viral incubation is 1-3 days).
Safe Ways to Equalize Pressure
Go gently — forcing it can damage the eardrum or push infected mucus deeper into the middle ear:
- Toynbee maneuver (safest): pinch the nostrils closed, take a sip of water, and swallow.
- Valsalva maneuver (very gentle only): pinch the nose, close the mouth, blow out gently through the nose — never blow hard.
- Yawn stretch: open the mouth wide and mimic a deep yawn while shifting the jaw side to side; a crackling/squelching sound is a good sign fluid is moving.
- Continuous swallowing: suck on a lozenge or chew gum to keep working the throat muscles that open the tube.
- Warm massage: during steam inhalation, massage gently behind the earlobes down the neck to encourage drainage.
OTC Options for Frequent Flyers
- Decongestant nasal sprays (oxymetazoline / xylometazoline, e.g. Otrivin, Nasivion): 1-2 puffs per nostril 30-45 min before descent. Fast-acting, but never use more than 3-5 consecutive days — risk of rebound congestion.
- Oral decongestants (pseudoephedrine / phenylephrine): taken about 1 hour before descent since they need to be digested; can cause jitteriness or a slightly raised heart rate.
- Pressure-regulating earplugs (e.g. "EarPlanes"): contain a porous ceramic/silicone filter that slows the rate of pressure change hitting the eardrum, giving the Eustachian tube more time to adapt. Insert before takeoff, may remove at cruising altitude, and must go back in 45-60 minutes before descent begins (not just when descent is announced) until after landing.
- Stay awake during descent — swallowing (which helps equalize pressure) drops off during sleep.
